Job Role & Responsibilities
As an HCC Medical Coder at Cotiviti, you will be responsible for accurate abstraction and coding of medical records in alignment with CMS risk adjustment guidelines. This role requires strong analytical skills, clinical understanding, and attention to detail to ensure correct capture of chronic conditions and disease severity.
Key Responsibilities
- Review and analyze outpatient, inpatient, and physician documentation for HCC risk adjustment coding
- Assign accurate ICD-10-CM codes based on provider documentation and CMS guidelines
- Ensure compliance with Medicare Advantage and risk adjustment regulations
- Identify gaps in documentation and escalate queries when required
- Maintain high coding accuracy, productivity, and quality scores
- Follow internal quality benchmarks, audit feedback, and compliance standards
- Stay updated with annual ICD-10-CM updates, CMS HCC model changes, and coding policies
- Collaborate with quality auditors and team leads to improve overall coding outcomes
- Adhere strictly to HIPAA guidelines and data confidentiality policies

Eligibility / Qualifications
This role is strictly for certified and experienced medical coders. Freshers without HCC exposure or active certification are not eligible.
Required Education
Any Life Science graduate background is acceptable, including:
B.Pharmacy, M.Pharmacy, Pharm D, BSc Life Sciences, BSc Biotechnology, BSc Microbiology, BSc Biochemistry, MSc Biotechnology, MSc Microbiology, MSc Biochemistry, BSc Nursing, MSc Nursing, Allied Health Sciences
Experience Criteria
- Minimum 1+ year of hands-on experience in HCC medical coding
- Overall experience range: 1.2 to 3 years
- Practical exposure to risk adjustment coding is mandatory
Certification Requirements
- Active coding credential from AAPC or AHIMA is mandatory
- Accepted certifications: CPC, CCS, CRC
- CRC certification is preferred for HCC coding roles
Skill Requirements
- Strong knowledge of Anatomy and Physiology
- Excellent understanding of medical terminology
- In-depth knowledge of ICD-10-CM guidelines
- Familiarity with CMS risk adjustment and HCC models
- Good written and verbal communication skills
